- Driver submits to tests with result equal to or greater than .08
- Driver either submits or must do Admin Per Se convicted or...
- Driver is convicted of DUI
- Driver does not have any prior DUI, Reckless Driving, or Racing offenses within the preceding five years
- Driver has not accumulated more then 4 MVD points prior to the incident
Arizona Motor Vehicle Division Activity
- 90 day Admin-Per-Se Suspension
- 30 days no driving, 60 days restricted driving
- Reinstatement fees
- Assignment to Traffic Survival School under MVD Driver Point System
- Ignition Interlock Required if convicted of DUI
